New Vision Provider

The Board of Trustees is pleased to announce a change in vision service providers. Effective July 1, 2020, EyeMed will be the Plan’s new vision provider. With this change you will experience the same great vision benefits, plus additional discounts from the EyeMed providers. You will have access to thousands of providers from independent eye doctors, your favorite retail providers such as LensCrafters, Pearl Vision, Target Optical, and many more. EyeMed also has online options available. To search for a provider, access the EyeMed portal at, “Find an eye doctor”, choose the “Insight” network, or try the EyeMed members App.

EyeMed will be sending you a “Welcome Packet” and an EyeMed ID card. To use the benefits is easy, choose an in-network provider, schedule your visit, and go in for care or eyewear. You do not need an ID card – let the provider know EyeMed is your vision provider and give them your name and birthday. As long as you stay in-network, EyeMed will handle all of the paperwork.

Delta Dental

Delta Dental of Ohio, with its affiliates in Arkansas, Indiana, Kentucky, Michigan, New Mexico, North Carolina and Tennessee, make up one of the largest dental plan administrators in the nation. In 2015, the enterprise paid out $3.3 billion for dental care for 14.1 million members.


Frontpath Health Coalition

Frontpath Health Coalition offers a large selection of PPO providers in the Toledo area. You can search for a provider by specialty, location, or other criteria.

They also have a Health Quality Improvement Program – Please click here to be directed to the login page. Once logged in you can run reports to differentiate cost and quality of medical facilities under the Frontpath Provider List.

American Health Holdings

American Health Holdings (AHH). All pre-certifications will be handled by AHH as well as emergency In-patient Services. Please call them at 855.248.1858 between the hours of 8 a.m. to 9 p.m.



The Plan contracts with EyeMed to bring you discounted services for vision care. The EyeMed website will allow you to locate network providers, determine when you are eligible for benefits, and tell you are eligible for benefits, and tell you the Plan’s benefits for network and non-network providers. You may also call EyeMed at 866-800-5457.



MassMutual customer service representatives are available Monday through Friday from 8:00 a.m. - 9:00 p.m. ET at 1-800-743-5274 to answer your questions and guide you through transactions.
