Frontpath Health Coalition

Frontpath Health Coalition offers a large selection of PPO providers in the Toledo area. You can search for a provider by specialty, location, or other criteria.

They also have a Health Quality Improvement Program – Please click here to be directed to the login page. Once logged in you can run reports to differentiate cost and quality of medical facilities under the Frontpath Provider List.

The Plan contracts with EyeMed to bring you discounted services for vision care. The EyeMed website will allow you to locate network providers, determine when you are eligible for benefits, and tell you are eligible for benefits, and tell you the Plan’s benefits for network and non-network providers.
